Chamber Wellness Initiative
New in 2010! Employers enrolled in the Chamber’s health insurance program can now take advantage of a wellness initiative which could result in higher productivity and lower insurance premiums for your company. The initial year of this program focuses on the completion of two core components:
1. Health assessment: A health assessment is a series of online questions regarding current lifestyle behaviors and medical status to determine risks for
developing chronic disease. Read more details now.
2. Health screening: During a health screening, participants undergo several tests to identify lifestyle risks, such as weight, blood pressure and cholesterol. Participants

Multiple health screening events will be held throughout
Northwest Ohio
to allow your employees the opportunity to complete this requirement. More information on dates and times will be listed here once they are scheduled. Additionally, employers that can assemble at least 30 individuals for their screenings may schedule an on-site screenings at no cost*. (*See additional details and participation threshholds here.)

This sample letter can help you draft a communication with your employees on what to expect and how they can complete the Health Assessment.
By meeting the minimum requirements for both the assessment and screening, employers that are enrolled in the Chamber’s health insurance program will qualify for a premium discount beginning with October 2010 renewals. For employers that meet the program requirements and have a renewal date prior to October 1, 2010, the discount will be applied to the 2011 renewal. Groups of 50 or less can earn a 3-percent premium reduction at time of renewal, and groups
of 51 or more can earn a 2-percent reduction (provided that the premium discount does not cause rates to fall outside the premium restrictions for small groups in Ohio.)
To qualify, the participation thresholds must be met for both components by April 30, 2010.
